Enable debug log level output and paste a complete session here (remove passwords, if any) and I'll see if I can get Deuce to come take a look.
In my sbbs.ini I have changed my loglevel to debugging so I'll see if I can get better info. I have one last log snippet from BINKPOLL with debug logging.
I did pick up an inbound when that poll ran and it was placed in my secure inbound so that makes me think the BINKP service thinks the session is insecure but the BINKPOLL event does see the session as secure and I don't see any mention of CRAM-MD5 of password does not match in syslog for this time frame.

No, there is only 00000001.clo and the listed .pkt files in there. I wonder if that .bsy file should be 00000001.bsy?
Deuce may be able to help more than I and he's not active in DOVE-Net right now, so you might have a better chance of getting support in #synchronet at irc.synchro.net.

I have never been an IRC person. Used it once around 1998! I've read that the chat section can be modified to connect to irc.synchro.net but haven't done that yet. It just went to the top of my todo list so I will try to catch Deuce there.

is it sending mail directly to the point in question or is it sending netmail routed through the points BOSS node?
I just committed some hopefully-helpful debug output to binkit.js. Can you update and retry and paste the output here (especially right before that ".bsy failed." message)?

There should be a lot more log output when full debug log output is enabled (e.g. M_NUL lines and such).
For incoming BINKP connections, those come into the service, so the log level would be configured in your services.ini file for those.

... and I think that outbound directory calculation is now fixed. I'm guessing you didn't actually have a "00000001.pnt" directory, but rather a "0004006a.pnt" sub-directory (off of outbound.015)?
